What is the difference between Routine Doctor visits and Non- routine doctor visit ?
I’m currently looking to buy a Health insutrance and iam comparing plans of united health care and specially around plans with HRA and the major difference between them is the Routine Doctor visits and non routine doctor visits. Can someone tell me whats the difference between the two?

It also means that your health care provider (the doctor you choose) submits your bill to our underwriting department. What is the difference between an office visit and an outpatient visit?
My doctor’s office is in a hospital and because my scheduled visit with this doctor was billed as an outpatient visit rather than as an office visit, my insurance would not pay for it. I’m confused as to what the difference is between an office visit and an outpatient visit. The appointment was scheduled far in advance and was in no way an emergency room visit.
